SUMMARY:The contribution of the JADECARE Joint Action in health systems  transformation: integrated care and digitization
DESCRIPTION:The objectives of the event are to disseminate the results of the project and foster its sustainability\, through a multidisciplinary approach and the active involvement of stakeholders and experts. The first session will be devoted to presenting the project\, its achievements and alignment with existing European\, national and regional policies\, programs and initiatives. During the second session\, a representative of the good practice will illustrate how the algorithm and dashboard are being used in the Spanish health care system while the project team will present the sustainability plan to implement regionally the results achieved. These talks will provide useful insights for discussion with experts\, stakeholders and participants in order to assess the potential for using the project’s products in the various areas of expertise and identify possible actions to be implemented after the end of the project. Below follows the preliminary program of the event. The event will be in Italian language. \nThe Marche Region\, through the Regional Health Agency\, is participating in the European Joint Action JADECARE\, aimed at addressing the challenges of healthcare transformation in the European Union. The 3-year project involved 45 organizations from 16 European countries and is going to finish in September 2023. In this context\, Marche Region implemented a good practice developed in Spain (Catalonia)\, focusing on chronicity management through digitalized systems. An innovative algorithm using administrative health flows to assess the health status of the Marche population and the distribution of chronic diseases has been successfully tested. The results of the algorithm will be made available through a dashboard that can explain and show the citizens’ health demand and support the planning of social and health care services. The management of chronic diseases will be promoted through integrated\, person[1]centered care\, allowing and efficient use of the available resources\, providing the quality of care\, and an improved quality of life for citizens.

SUMMARY:Implementation Research Scientific Conference
DESCRIPTION:The Implementation Research Scientific Conference in Bilbao comes at a crucial juncture in which it is essential to think about how Implementation Science and practice may work together in order to facilitate the translation of evidence-based interventions into routine clinical practice aiming to increase their reach\, adoption\, implementation and maintenance.  \n\nJADECARE has been accepted as a presenter to this conference on “The Implementation strategy for the transfer and adoption of successfully proven good practices on digitally enabled integrated person-centred care to heterogeneous contexts”: In the framework of speeding up the adoption of integrated person-centred care solutions in Europe by improving their capacity to redesign and improve their healthcare systems\, the EU funded “Joint Action on digitally enabled integrated person-centred care” intends to support the transformation of health systems by addressing the transfer and adoption of four “original Good Practices” (oGPs) to 21 settings of “Next Adopters” (NAs) from 14 European countries holding different levels of maturity of integrated care. A three-step implementation strategy has been developed for enabling the transfer of experiences and knowledge: \n\nPre-implementation (planning and preparation): the NAs analyzed their contexts with the SWOT analysis and identified more than 150 local needs aligned with oGPs‘ key elements (study visits organized). As a consequence\, 64 interventions were defined targeting more than 4 million people.\nImplementation (roll-out and operation): the NAs deployed their action plans by means of two PDSA Cycles monitored by more than 350 KPIs. In ten structured thematic workshops they exchanged their experience about the transfer process.\nPost-implementation (impact assessment and learning): the NAs analyzed the implementation process through the CFIR and reported the whole experience using SQUIRE 2.0 guidelines. Moreover\, the implementation strategy is assessed by NAs (end users) to evaluate its impact and usability.\n\nJADECARE impacts on European health systems by implementing innovative digital integrated care solutions and helping to change the model of care provision. It sets the base to implement integrated care at large scale by delivering know how to transfer innovative and tailored practices and generating evidence that aspires to produce benefits beyond the project. \nImplementation Research and Practice:The biggest problem that undermines the quality of our health services is not the lack of research\, but rather the lack of real implementation of the knowledge that research generates. The real translation into clinical health practice of effective interventions for health promotion and prevention of chronic diseases remains an unresolved challenge in health systems worldwide. Additionally\, a considerable proportion of the interventions provided to patients in the healthcare context lack solid evidence of the effectiveness. The elimination or reduction of this low-value care can lead to improvements in the quality of care and its results in terms of population health while reducing costs. \n\n\n​Implementation Science seeks to address and reduce this translational gap by studying and understanding:        \n\nThe political\, social\, behavioral\, economic and organizational factors that impede the effective dissemination and implementation of clinical\, public health and organizational findings based on evidence.\nThe identification\, development and evaluation of specific strategies that allow the integration into routine clinical practice\, interventions and therapies of proven efficacy that are not being applied in a generalized\, sustainable and continuous way (Implementation) or promote the abandonment or substitution of ineffective or cost-ineffective practices or interventions (De-implementation).

SUMMARY:European Implementation Event EIE2023
DESCRIPTION:At the #EIE2023\, Implementation – the translation\, uptake and integration of research findings within the daily routines of human services – takes centre stage. Attendees will learn about latest\, cutting-edge implementation science and practice; participate in inspiring and surprising discussions with international colleagues; expand your professional network; and be inspired by the diversity of this exciting discipline. \n\nEU funded Joint Action JADECARE\, aiming to assist Member States in undertaking health system reforms by supporting the transfer of four “original Good Practices” (primary care centers\, hospital and community setting) to 21 “Next Adopters (NAs)” of 14 different EU countries will be hosting a workshop at EIE2023 demonstrating its implementation strategy.\nThe three step implementation strategy has been developed to enable the transfer process that includes a series of techniques\, concrete procedures\, guidance and recommendations.\nThe three main phases are: Pre-implementation (planning and preparation)\, Implementation (roll-out and operation\, based on PDSA cycles) and Post-implementation (impact assessment and learning).  The strategy provides a blueprint for adoption\, implementation\, monitoring\, reporting and sustainability of successful interventions into new contexts.  Furthermore\, the successful use case of JADECARE sets the base to implement digitally enabled integrated care at large scale\, which translates into introducing innovations in health systems that result in better health care.\nFollowing the presentation of its strategy\, JADECARE representatives will discuss two key questions with the audience:\n1) How relevant is it to focus on the prior preparation of the local environment and to consider the key contextual determinants of the implementers to modulate the success of the implementation process?\n2) How can a community of learning stakeholders be promoted that explores ways to develop\, collect and exchange knowledge and performs concrete action for boosting and leveraging the sustainability of the implemented practices? \nEIC overarching goals\, also pursued at EIC2023  are to: \n\n\n\nBe the “go to” network for knowledge and resources on European implementation science and practice.\nConnect individuals\, groups\, networks\, and organisations\, reducing the implementation science – practice gap.\nActively promote equity in implementation science and practice.\nCollaborate with organisations\, networks\, and groups outside the EIC to push forward implementation science and practice.\n\nThe European Implementation Collaborative (EIC) aims to build a connected European community of practitioners\, researchers\, and policy makers with contemporary implementation skills and expertise\, improving the quality of life for all people.

SUMMARY:23rd International Conference on Integrated Care (ICIC23)
DESCRIPTION:The 23rd International Conference on Integrated Care (ICIC23) in partnership with the Flanders Agency for Health and Care and Visit Flanders will take place at Flanders Meeting & Convention Center Antwerp\, Flanders from 22-24 May 2023. With the overarching theme ‘Care in action: how to work together\, a participatory approach’\, the conference will bring together leaders\, researchers\, clinicians\, managers\, citizens\, patients and caregivers from around the world who are engaged in the design and delivery of integrated health and social care. \nJADECARE will be present at the event in the format of a dedicated JADECARE workshop\, as participant and contributor in an EC hosted workshop for EU funded projects and an oral presentation by one of the Next Adopters (Andalusia):\n \nJADECARE has been accepted for hosting a workshop under the objective to share the results of the implementation process of JADECARE and explore the key elements to ensure the sustainability of local good practices beyond the Joint Action. In this regard\, the results of the implementation process of JADECARE will be presented and the key elements to support the sustainability of good practice transfer beyond the Joint Action will be explored. Furthermore\, the workshop presents the JADECARE Sustainability strategy that sets three core elements: policy environment\, ownership of sustainability and culture of collaboration and consensus seeking. In addition\, two Next Adopters (NAs) will present the results of the implementation of their local good practices and their work to build sustainability beyond JADECARE. Finally\, discussion  among attendees will be promoted to highlight the key elements to ensure sustainability and scale up of local good practices beyond the project.  \nIn addition\, JADECARE has been lately invited for participating and contributing in a workshop on learnings by EU funded projects (hosted by European Commission).\n \nMoreover [in Session 4.1 Care Transitions]\, the JADECARE Pilot in Andalusia will be presented: Improving healthcare at home for complex chronic patients (CCPs) in Andalusia. Abstract: A Complex Chronic Disease is a condition involving multiple morbidities that requires the attention of multiple health care providers and\, therefore\, a holistic approach. The Andalusian Public Health System is responsible for the provision of healthcare and public health services to the Andalusian population (8.5 million inhabitants)\, where around 200000 complex chronic patients (CCPs)\, with chronic heart failure (CHF) and chronic obstructive pulmonary disease (COPD) have been identified and prioritized. Healthcare at home for CCPs is considered a key element that facilitates patient follow-up\, can avoid emergency episodes\, promoting patient empowerment. The Andalusian participation in the EU Joint Action JADECARE “joint action on implementation of digitally enabled integrated person-centered care” (GA 951442) focuses on improving healthcare at home for CCPs with CHF and COPD. The development\, implementation and testing of a Centralised System for Proactive Follow-up (SCSP) of chronic patients is being monitored\, as well as the tele-referral service which is being expanded. The SCSP is supported by National funds as well (Servicio para la implantación de una solución corporativa para el seguimiento proactivode pacientes crónicos en el sistema sanitario público de Andalucía (Expdte. 039/21-SP.). DESCRIPTION:Upcoming (free) JADECARE Webinar “Taking care of the complex patient: the contribution of Joint Action on implementation of digitally enabled integrated person-centred care”: The increasing prevalence of chronic diseases and the frequent presence of multimorbid conditions represent a public health priority and a shared challenge present in both the Italian and European contexts: providing efficient\, personalized and integrated care that promotes better quality of services is key to address this challenge. \nTo this end\, the Regional Health Agency\, on behalf of the Region of Tuscany\, participates in the European Joint Action “JADECARE” (Joint Action on implementation of Digitally Enabled integrated person-centered CARE) whose purpose is to contribute to the development of more innovative\, effective and sustainable health care systems by supporting the sharing of some “Good Practices” already in place in other European countries. Therefore\, in accordance with the objectives of the project\, professionals from ARS Tuscany and “Piana di Lucca” District Zone have opted to transfer and implement some components of the Good Practice of the Basque Country in “Piana di Lucca” site. As a result\, a model of taking care of the complex patient that involves the co-presence of a multiprofessional and multidisciplinary team has been outlined and implemented in the “Piana di Lucca” District Zone. \nIn this second event\, which follows the Webinar on presentation of the project that took place in October 2021\, the Regional Health Agency’s objectives are to: \n\nProvide an overview on how the JADECARE project can fit into national and regional health policies\nEmphasise the Tuscany Region’s commitment to supporting European planning\nUpdate health professionals on the status of the project in “Piana di Lucca” District Zone: key point of the implementation\, main results\, achievements\, barriers and challenges\nUpdate health professionals on new “branching activities” in the South-East and Central Local Health Authorities: both Local Health Authorities have shown interest and willingness in transferring into their settings some components of the project developed in the “Piana di Lucca” District Zone.\n\nRegister here

DESCRIPTION:The conference scientific committee invites submissions of abstracts (in English language only) for presentations related to the scope and purpose of the conference. Please check the list of EFPC 2022 Ghent Conference – Themes. \nDeadline for submission: 1 May 2022 \nSubmissions should follow one of the following formats: \n\nResearch abstracts: These presentations will provide the results\, completed or in advanced in-progress form\, of original research projects. Papers related to PhD projects\, either completed or in progress\, are especially encouraged. The selected abstracts will be presented via a max 12 min powerpoint presentation and discussed within workshops that are moderated by key-experts in the specific theme.\n\nPolicy debate posters: These posters describe significant developments in policy (whether governmental\, organisational or any other) that affects primary care associated with the themes of the conference. In particularly national\, regional or local policymakers are invited to provide new policy developments. Posters need clear statements for debate. All selected posters will be presented/defended in specific parallel sessions with a 15 minutes time-slot for each poster. These sessions are located in innovative settings\, such as “debate corners”\, animated by an expert facilitator.\n\nVideo presentations: Projects and developments focusing on practice-oriented questions and reporting on recent experiences and innovations in primary care. These presentations should present case descriptions of primary care on the national\, regional or local level. The selected videos will be displayed prominently with non-stop shows during the conference (length should be around 3 minutes and not exceeding the maximum of 5 minutes) and will be presented/defended in specific parallel sessions with a maximum of 15 minutes time-slot for each video.

DESCRIPTION:JADECARE will be represented */** at the 22nd International Conference on Integrated Care which will take place in Odeon in Odense\, Denmark\, from 23 – 25 May 2022.  The conference is a partnership with Healthcare Denmark in cooperation with Odense University Hospital\, University of Southern Denmark (SDU)\, Municipality of Odense\, Campus Odense and Destination FYN. Denmark is among international frontrunners when it comes to integrated healthcare services. \n\nA coherent and integrated healthcare system is key to solving the demographic challenges and reduce patient length of stay at hospitals. A high degree of coherence means that Denmark can address newly diagnosed cancers with clinical pathways\, which connect hospitals\, clinics\, GPs and the patients. This has already led to remarkable progress in Danish cancer survival.\nA coherent and integrated healthcare system with a high degree of digitalization enables real-time sharing of electronic data and makes it possible to initiate early diagnostics and address chronic diseases in new ways\, where the patients are empowered and involved in handling their own disease.\nElectronic communication and sharing of information enable municipalities\, home care staff and the hospital to support older people to stay in their own homes as long as possible. As a result\, the patients experience fewer admissions and shorter length of stays at hospital\, and the risk of frailty\, leading to long hospital admissions\, is drastically reduced.\n\n* JADECARE – SCIROCCO Exchange – VIGOUR “A workshop to explore the challenges and opportunities for transferring good practices\, build capacity and scale up integrated care” (Monday\, 23 May\, 13:00 – 14:00\, Lunch Session MLW.1): The Joint workshop will facilitate the discussion to analyse the challenges and opportunities of transferring Good Practices by these projects providing technical assistance to health authorities in transferring\, developing capacity and scaling-up integrated care under the EU Health Programme: \n\nJADECARE Joint Action reinforces the capacity of health authorities to address the transition to digitally-enabled\, integrated\, person-centred care. Four “Early Adopters” of original Good Practices (GP) support transfer of features into the systems of 21 “Next Adopters”.\nSCIROCCO Exchange supports the readiness and capacity of health and social care authorities for the adoption and scaling-up of IC by developing a Knowledge Management Hub.\nVIGOUR backs health authorities on their systems transformation by means of a context sensitive support programme guiding each authority through staged process of IC ambition focussing\, maturity self-assessment and operational implementation planning.\n\n** JADECARE Workshop “Transfer and adoption of digitally enabled integrated person-centred care good practices: which challenges and key elements need to be addressed for a successful transfer?” (Tuesday\, 24 May\, 16:30 – 18:00\, Parallel Session 5i): This workshop explores the challenges and key elements to be addressed when transferring Good Practices (GP) to heterogeneous health systems with different needs\, aims and scopes. It features the core elements of the four GP of JADECARE (Joint Action on implementation of Digitally Enabled integrated person-centred CARE)\, concerning integrated care\, chronic conditions\, self-care\, prevention and population health that are being transferred and adapted to the contexts of 23 Next Adopters (NA). GP are: Basque Health strategy in ageing and chronicity: integrated care (Spain)\, Catalan open innovation hub on ICT-supported integrated care services for chronic patients (Spain)\, OptiMedis Model-Population-based integrated care (Germany)\, Digital roadmap towards an integrated health care sector (Denmark). It will also highlight the experience of two NA: Portugal and Estonia. \nThe conference will operate as a hybrid event. While we are very excited to be physically together in Odense\, the hybrid formal means that people who do not wish to travel next May can join and present at the conference via video link and present their paper digitally\, however a delegate fee and registration will still be required.

DESCRIPTION:UPDATE: According to the European Association of Systems Medicine\, the 3rd EASYM Conference has been postponed to Spring 2022. The precise dates of the event\, to be held in Barcelona\, will be announced. \n(maintained were 2021 EASYM ACADEMIC EVENT & GENERAL ASSEMBLY: The training program (tutorials) run on 9th November 2021 from 09:00 to 16:30\, followed by one-hour academic session and the day ended with the General Assembly) \nEASyM 3rd Meeting 2022 (postponed from 2021): “Systems approaches to Global Challenges”: EASYM (European Association of Systems Medicine) was founded in September 2015 and therefore celebrated its 5th birthday last year. Many things have happened in these past five years. The Systems Medicine approaches have been broadly incorporated in the academic arena and its principles are progressively embedded into ongoing translational research programs. They have led to a better understanding of disease mechanisms\, as well as to better capture the nature of disease interactions\, paving the way for new therapies. Moreover\, Systems Medicine is contributing to define novel strategies aiming at optimizing health value generation. \nFor Systems Medicine to be able to radically change health care still large steps have to be taken. This is why EASYM thinks it is important to bring together all stakeholders in the field (biologists\, computational scientists\, clinical researchers\, practicing clinicians and managers) to discuss and work together on methodology\, interpretation of data and implementation of results in clinical care. \nTo be able to bring these stakeholders together\, EASYM considers pivotal to organize a 2-yearly networking event with a strong program to inspire and to give people the opportunity to discuss and brainstorm together. Furthermore\, EASYM wants to play a role in training scientists working in the field of Systems Medicine and educating clinicians to promote clinical practicalities of the approach. SUMMARY:The VIII Digital Congress on Digital health\, "The San Sebastian International Digital Health Congress (e-health Donostia)"
DESCRIPTION:The San Sebastian International Digital Health Congress has been held continuously since 2014. In 2021\, its eighth edition approaches the theme of Value-Based Digital Health: The recent health crisis has caused a global crisis\, affecting society in all areas. We must take advantage of what we have learned to consolidate value-based practices and address the opportunities for improvement that have been identified. \nOne of the priorities identified was undoubtedly the need for a digital transformation in health. Digital Health is a reality in our environment\, but there is still a long way to go. We must reflect on whether the current scenario is adapted to the present and future needs of users\, and what would be the most appropriate strategies to meet these needs in the coming years. The new virtual format of the congress since 2020 facilitates the attendance of people from all over the world\, allowing the sharing of experiences and lessons learned from different environments. By working together\, we can move faster and go further.

SUMMARY:ICIC21 “Realising the true value of Integrated Care”
DESCRIPTION:In 2021 the 21st International Conference on Integrated Care (ICIC21) “Realising the true value of Integrated Care” took place as a virtual conference\, co-hosted by IFIC hubs and collaborative centres IFIC Scotland\, IFIC Australia\, IFIC Ireland\, IFIC Canada and IFIC Latin America. The conference ran across the whole month of May taking place across various time zones. \n\n\n\n\n\n\nPresenters pre-record their presentations and were available to all delegates to access in their own time. During the live sessions successful authors were asked to reduce the time of their presentation to key points and sessions were ran interactively hosted by two or more chairs leading a discussion with paper presenters. This was much more enjoyable for the viewer and more engaging from the audience for presenters by using this format. \n\n\n\n\n\n\nWorkshops were designed with the virtual audience in mind and for making the best use of digital technology and provides the best learning experience for attendees.
